Important: This document MUST be signed in the presence of a Slapshots Sportplex employee or be notarized by a commissioned notary prior to addmission to the park. In addition, users of our Extreme Park must wear the following protective gear: helmet, wrist guards, elbow pads and knee pads. Slapshots Sportplex DOES NOT rent this equipment.
